If deficiencies are extensive or systemic:

Compliance Tracking System Implementation

If an agency does not currently have a structured compliance tracking system in place, Back Office Care can assist with implementation.

Includes:

• Expiration tracking framework
• Caregiver credential monitoring system
• Background and fingerprint compliance structure
• Training documentation tracking
• Audit-ready file organization system

Back Office Care may conduct structured caregiver outreach for missing documentation. If documentation cannot be obtained after reasonable outreach attempts, the matter is escalated to agency leadership.

Flat Fee: $495

  • Credited toward full assessment

Survey Readiness Assessment
Flat Fee: $1,500

  • Up to 10 caregiver files reviewed
  • Up to 8 client records reviewed
  • Policy & procedure alignment review
  • One summary report with findings + recommendation
  • One follow-up clarification call (30–45 min)

Most agencies complete a readiness assessment once during each survey cycle.

Agencies with prior citations or complex compliance gaps may require expanded review pricing.

Citation / Corrective Action Support (CAP)
Starting at $2,500
Scaled based on number and severity of findings.

  • Up to 5 citations/deficiencies addressed
  • CAP framework + documentation guidance
  • One revision round per citation
  • Final review before submission

Monthly Regulatory Support Add-On $300/month

  • Up to 5–8 hours of support/month

Scope Limitations

Back Office Care provides operational support and does not:

• Provide legal advice
• Represent agencies in legal proceedings
• Guarantee survey outcomes
• Override agency employment decisions
• Act as employer of record

Additional documentation review or extended support is billed separately.

All final regulatory accountability remains with agency ownership.
